You should find the image in the path themes/default/images/ajax_preview.png. If it's not on your webspace it should be in the ZIP-file respectively the unpacked directory.
If you find it wether on the server nor in the local copy or ZIP, you can copy it from this forum (right click on the image=>save as).

There seems to be another problem, but none with the image in itself. It might be a browser plugin, that is triggered because of a part of the file name?
