I am not aware of any reason, why this should happen. The favicon is declared in the main template, which is used for most of the forum pages with exception of a few documents like the upload-popup.
Did you change anything like the theme? Did the URL in the HTML-meta-element, wich points to the icon, have the correct URL (typically something like "http://www.example.net/forum/themes/default/images/favicon.ico") in the bothered pages? Is the protocol of the URL correct ("http://" vs. "https://")?

Just seems to happen when using inPrivate browsing yet all other websites I go to show their favicon just fine on all pages using inPrivate browsing.
I did not change the theme, it says in the header element on affected pages
<link href="http://mydomain.com/themes/default/images/favicon.ico" rel="shortcut icon">
Going directly to
http://mydomain.com/themes/default/images/favicon.ico
shows the favicon

Ok.

Ok, so you use the original theme. It's the original favicon.ico too or did you changed that file?
This is the original one (in Firefox):

Yes the original theme and original favicon as your picture. I have not modified anything.
